Unsuccessfully Integrate LinkedIn Login Authentication

Hi,

I'm trying to integrate LinkedIn Login Authentication to my app. Although I've successfully done for LinkedIn login page, but after clicked on sign in and unhandled exception pop out, and I cannot find the prevForm.show in my code. Please help me on solving this issue.

Below screenshot is the error that I got after login. Attached document is more details on the setup of LinkedIn Auth.
