I tried to use the “O” to create an “OU” but keep getting the error prompt “Entry not found in index”. However, I tried to use the same “O” on another machine running on the same lotus notes version, I’m able to create without any error.
I already tried to reinstall but still, the same error appear. Could it be my ID or some of the NSF file is corrupted?

It sounds like your registration server was set to local. Make sure you have it set to a server.

I have set the Registration server to the correct Domino server.
Manage to resolve it. What I did is to remove all the certifier name in my personal address book! I suspect it may be corrupted when I created a few test O & OU.
