Our management has asked that personal data, such as home addresses and home phone numbers, in Notes address books be hidden from viewing to anyone without specific access rights. We need this for both Domino Directory and address books based on the pernames.ntf template, which we use for shared address books . Looking at the template design, it seems that this data is part of the contact form, rather than part of a subform.
Perhaps the least intrusive method would be to use the Extended ACL of the Domino directory. With the EACL you can restrict fields to only certain people or roles. It is better that using hide when formulas because it removes the ability to see the value from document properties.
Personal Address Book in Notes Client should be protected if your users are logging in using an ID file/ password. If they are careful enough to lock Notes while the workstation is un-attended, no one should be able to access their PAB. However, if it is a shared workstation, then it’s a diferent story.
Secondly, so far I have not experienced having users personal info added in the Domino directory (even though the fields are available).

The problem is not with an individual PAB (names,.nsf) but with shared address books based on the pernames.ntf template.
Since there is not much documentation available for this template, (nor for pubnames.ntf), it is difficult to modify the template or databases based on this template to ensure privacy without worrying about breaking the functionality.
It would be nice if Lotus provided documentation for pernames.ntf like they have done for the teamplan template.
