We have configured the Out of office to run as a service on a 8.5.1 server. It is supposed to answer one time to a sender ( This is the only notification you will receive while this person is away. ) but it replies two times.
Is there a fix ?

There is a system message at the bottom of the message which says that it will reply only once.Does your message say that, or does it say it will reply to every message?

as written in the title … just once and it replies … twice ! not every messages.
Subject: okay. theory number one is not the one!
The was an issue in the template was corrected that had problem, but the message at the bottom would have been different.
Theory #2: do your user use iNotes? There was an issue when the the state could get out of phase when the user used iNotes to enable/change OOO.
