Since a couple of days, my Notes client (standard 9.0.1 FP2) starts with a none standard dialog box that just ask for the Notes password.
This box is longer but the original one but less large :

Subject: Search notes.ini for a line similar as extmgr_addins=extpwd
The above line (or similar) gets added to notes.ini by programs that hook into the Notes login; see for example the following article:
