You can:
Send an email from your mobile. Email adress is mapped to your database. This is the easiest solution to setup on server, but not all mobile phones has email features.
Send an SMS message to a special phone number which forwards the message as an email. Works on all phones but requires an SMS2Email gateway.
Use mobile phone’s WAP browser to open a Domino web page and input data into required fields. This can be done with software provided by Lotus or with your own solutions. Something like this: Botstation - WAP NAB Examples
Many modern phones have full-featured web browsers whgich can be used to open regular web pages and put data into fields.
