How to send automatic survey request in Connections Cloud

How to send an automatic survey request to our customer when their entry is completed and closed in in our community (Connections Cloud). It sends an automatic message informing them that the request is closed, but how it can be modified to include a survey question

Are you talking about the automated notifications which gets sent when the Activity entry is complete and you want to add a survey question to the email notification ?

Hello Aditya,
Yes I am talking about the automated notification. If it can be customized for our needs and to send a survey request. If here is another way to create a survey that will be sent to our customers when their request/question is closed.
