How to populate a .pdf with data stored in notes?

I’m trying to populate a .pdf form with data from Lotus Notes 6.5. I scan in a form and added fields. I would now like these fields to be populated with data stored in Lotus Notes.

I have done this in the past but can not remember what was done to accomplish this task.

I have used this iText jar file and it produces a good pdf and that too free.
