When I try to add a CONFIGURATIONS Document (Servers > Configurations) it disappears immediately after I create it.
All was working very good then this began to start happening.
I now can no longer send outbound internet e-mail.

Did you use webadmin to create the config doc? If so, this is a bug and is fixed in R6.03 and R6.5.

The only time I’ve seen a config doc disappear was under R5 if I tried to block a domain (in the mail router specifications) that had a dash in the name.
However, what I suspect is going on in your case is a replication issue where another replica (especially if it’s in a cluster) is wiping it out. I recently had a similar problem where Server Rules (also part of the Server Config doc) would disappear immediately after creation. I had to make sure all of my servers had the correct date/time set, force a replication among all of them, then purged deletion stubs in my NAB. Afterwards, I could create my Server Rules and they wouldn’t get wiped out by other cluster member(s) after saving.
