When I initiate a rename of a user, I receive the following error:
Title: [Company] Address Book File name: names.nsf; Error: One of the names to be added to the directory already exists in a different entry in the directory.
We only have one admin server and one directory. We have renamed people via the adminp without problem for some time, but now we are unable to. I have restarted the server and this did not help.
Any suggestions on what to check?

You could try a search (using the search bar) for the new name you are trying to use - this will tell you if there really is a conflict

I just checked again to be sure, and the name is not present.

I have run into the same problem.Creating a view in Names.nsf showed no other documents like the name i wantet to give a person.
Actually this was a rename back to a previously used name.
I found that the server document pointed to a directory assistance database and here I found an old record with the name I wanted to use. By renaming this document I was allowed to make the wanted rename of a person.
