FYI, 8.5.2 has not yet been added to the Release field above.
I am running Windows 7 64 bit. The icons appear black after a clean install of 8.5.2 gold.
Attachments for received email look generally ok (unless it is those graphic icons from the Notes letterhead that come across when other Notes clients send to you through the Internet). Attachments added to a new email are all black whether using drag and drop or using the attachment paper clip.

I also observed this issue - using Windows 7 64bit. After I refreshed (replaced) the design of bookmark.nsf (and also names.nsf) on the local client, the problem went away for a while. However, it later returned again, albeit showing a different icon then (Blue Windows 7 Aero “Busy” mouse pointer on black background). Meanwile, I observed the problem to be intermittent.
I’ve documented the issue as SPR # RSCR88GND8. I recommend you open a PMR with reference to this SPR.
Happens on 32-bit as well, and the image icon isn’t always a small black box.
I’ve got three forwarded emails with attachments in my inbox, one of which has a 200x200 black box, one that has a copy of the user’s taskbar as the icon image, and another that has a massive 3120x1050 copy of the user’s multi-monitor desktop!
There must not be a way to directly ask Windows for the icon resource corresponding to a particular file, so Notes must be creating a hidden window, asking Windows to display the icon there, then capturing an image of that window to use as the icon. Only sometimes it must grab the wrong window ID.

Happens to me also, When I attach a file, it shows weird icons, not the one for that file type, e.g. not an excel icon when the file is an MS excel spreadsheet.I am using Notes 8.5.2 GA and Win7 Prof. x64
I’m getting the same issue… I tried to refresh the design locally and still doesn’t seem to help… Just seemed to happen all of the sudden… Mine is just a long multi colored bar
I have a user that has the desktop background image as his attachment icon (which is huge). This is really causing a problem now. He is using Windows 7 32bit and 8.5.2 client. I have tried the Refreshing bookmarks and address book, but it didn’t work.
I did a refresh design of bookmarks and names and that seemed to do the trick for me. The icons are now showing up properly. Now, we’ll see if the problem comes back.
Subject: Problem solved the problem by adding parameter in notes.ini
I had the same problem and I solved the problem byadding “AttachmentsUseCaption=0” parameter to Notes.ini in LN client just like Rene Thorarinss from previous posts.
Thanks very much. This fix solved the user’s problems here. AND SP1 and SP2 do not fix this…I have tried and it still does not work until we do this INI file fix…
Subject: Same problem experienced in Release 8.5.1FP5 SHF29
I am experiencing the same problem (icons with black circle over black square) in a new install on a new laptop on Win-7 64bit using Notes Release 8.5.1FP5 SHF29. It appears that the AttachmentsUseCaption=0 option added to the Notes.ini file did correct new instances of teh problem, but does not address notes that already included the issue.
SPR# RSCR88GND8 : Intermittently, in some Notes 8.5.2 Standard clients running on Windows 7 64-bit, an incorrect image displays for all attachment icons. Typically, the incorrect image that displays resembles one of the following two icons: a blue green circle inside a black square or a black circle above a black square. This is a regression introduced in 8.5.2.
