Try running: from the data-directory:
nfixup -F names.nsf
nupdall -R names.nsf
ncompact -c -i names.nsf
If that doesn’t resolve the problem, open the “names.nsf” Database using a different Notes client and check the $Programs view for extraneous Program Documents.

Subject: Program View
I can’t found the Program View. Mind to tell me where is it? Thanks
Subject: Re: $Program View
Hi Nick,
To locate the $Program View, follow the steps outlined below:
-
Open your Personal Address Book (local names.nsf)
-
Press and hold Ctrl+Shift. With those keys held down, select View–>Go To…
-
From there, scroll down approximately Three Quarters of the way down and select “($Programs)”
Also, oddly enough, I have seen where this error has been resolved by rebooting the Domino Server.
I hope this information helps Nick - Good Luck!
Regards,
-Bob
