What are some reasons that can cause a $Ref field to disappear and no longer link to a parent doc ?

One reason is that a response document can be created in lotusscript with any form. If the form is not set to be a response document in the form properties then the next time it is saved it will revert to being a main document and the $Ref field will disappear.
