Traveler NTSDB fragmentation

Traveler status today:

The IDX_TS_RECORD_MAP_2 table or index has a large number (17,157) of pages allocated that are unfilled. You should perform a defrag of the NTSDB at the next restart of Notes Traveler to reduce database space and improve performance.

A very similar status with the same recommendation occurred less than 3 weeks ago. At that time, I followed the steps to defrag the NTSDB.

What causes this condition, and how can I keep it from recurring so frequently?
