For some reason we have a user that is getting her rename reversed about every two weeks. what is the best way to stop this from happening???

If you’re also running 6.51 on your servers the best solution is to upgrade to 6.56 (or at least 6.54).
When you do a rename the server is expecting the user’s client to post to the AdminP database when the change was accepted. Priot to 6.54, if that acceptance wasn’t recorded the name swould be changed back. In 6.54 and later they new name stays in place, and you’ll get a new AdminP request to revert the name which you can approve if you want to.
