Renamed user is reverting back to Old after somedays

I am facing a strange problem.

I am renaming the user or changing his registration from one location to other location with following procedure

Open Administration Client

People & group ->People-> Select User-> Right click rename

After that its generates admin request. Then I am approving this requesting by going to administrator request. (Server-> anylisis → administration requests.

This is worked fine but after some days (15/20) it automatically getting changed to Old status.

Can anybody help me out what is going wrong? or my method is wrong? or i am missed out some steps?
