Print on readymade design document

Hi All,I have a document(form) where 4 fileds are there.I have a ready made worddoc sheet with design.

Now i have to pull the data of those 4 fields in different place of the readymade doc . For example,

Sir “fieldName”,

This is your enquary number"fieldenquary".

Thanks.“usermailidfield”.

“fieldName”,

“fieldenquary”

“usermailidfield”… these are the fields in the form (document) .There are a button ‘Print Quote’ and after clicking on this button only these three fields value will be print on the formatted document.

I think this is enough idea to get my problem.If you need more information i will supply.

Subject: print on readymade design document

The simplest solution is to export the list of fields in docs in a comma delimited file and let WORD handle the multi doc printing.
