Outlook allows definition of multiple email addresses to the same person (for example JoeBloggs@company.com and jbloggs@company.co.uk) and all incoming mail gets correctly delivered. Other than having secondary PNABs, can this be done just in the user document? I tried changing InternetAddress field to allow multiple values, and they show in view $Users, but mail to the second address doesn’t get there.

You can have multiple Internet email addresses under certain circumstances. One, if you have multiple Internet domains configured globally and do not have exact name matching turned on users will be able to receive email to address such as john.doe@domain_a.com john.doe@domain.b.com etc,. Where the username is static and the domain is one of your configured domains.
If you wish to use a different username then you may configure alternate email address in the User Name field in a manner such as john.smith@domain_a.com and so on.
