Mail template not always upgraded after SmartUpgrade

I am using SmartUpgrade to move from Notes 7 to Notes 8.5.1. I also have an organizational policy and desktop setting that upgrades the mail template. I have tested the SmartUpgrade successfully numerous times on a certain computer and have had a handful of pilot users run the upgrade. Most of the time, the mail file gets upgraded. A couple of times it has not.

Then, at some point after the software upgrade, the user will receive a message:

Your administrator has specified that your mail file design should be upgraded. The upgrade will begin immediately.

Click Ok and two messages appear:

Upgrade of mail file failed. Operation stopped at your request.

and

You cannot quit Notes because a dialog is active…

I can’t pinpoint any different with these two installs. I’ve checked the ACL, the users are both on the same mail server as other users whose mail file was upgraded.

Any ideas?

I get the same thing on my test PC and account. Until I find a solution, I can’t use this for real users.
