Hello, I have a Suggestion Database with a form to get information from our company’s Internet, then the manager use Note to view the informatiom. IMy question is how to make a URL to the Internet that link to my database form? what the Access should give the users to create and submit the form? users do not need to review and edit in the web site.

First of all you’ll need to make the form workable for both Notes and the Web or you could run into issues with either the Notes Client and/or Web.
With that said, you can link to any form over the web by doing a link something like this:
http://www.domain.com/dbname.nsf/FormAlias?OpenForm
If the database requires authentication you’ll have to give users a login to create this form. By default the form is open to all authors and higher, so you can simply give the user(s) author access with “Create Documents” selected in the ACL.
This will give them access to create a document, but they won’t be able to review or view it again once it is submitted unless you throw on an authors field with that user in it.
