Form Not showing

Im sending an email with an agent and with a form i created. I checked the option to have the form stored in the document but when people recieve it there is nothing. But if i do a reply with history im able to see my note… any idea how come?

Does your form have hide-when formulas?

Also if i send the email to my gmail or hotmail account

Im able to see the data…

not hide formula i just checked and it isnt checked

so it shouldnt hide…
