Extending SOAP header of webservice

I received a WDSL file for building a Webservices consumer. The WDSL file is fine, however an additional specification says that I have to extend the SOAP header with a username, password and a customerID.

Does anyone know how to extend the SOAP header. I am free to use Java or LotusScript in the Designer.
