Error when approving widgets

When I click on Approve in widget document, I get the following error:

I got some LS errors in the Click event handler of the Approve button. I had to remove all script libraries in the database and replace the design, after that I was able to approve the widget.

Would like to know a little more about exactly what happened here.

  1. What template was the Catalog using when the problem occurred (i.e., a beta template or the 9.0 Release template)?
  2. What template to did you use to “replace the design” to fix the problem (same template as 1. or different)?
