Error creating Desktop Settings

I got a problem creating just Desktop Settings for a policy, when I want to create it appears the following message:Error, the form and subform both have a fild entitled $qual_001

I created all other types of Settings, but just in Desktop i get the error.

somebody can help me please?..

download the template pubnames.ntf and replace Domino directory’s design.
