I’d like to enter the following code to allow the Phone Message icon onto Lotus Notes but I don’t know how to do it. Can anyone help?

Assuming you want a smart / toolbar icon:
File > Preference > Toolbar Preferences …
Click on the Customize tab
Click on the New > Button
Fill in the dialogue box: Caption Text: Phone, Popup Help: Phone Message, Formula is what you posted. And pick your icon.
Click on the green checkmark and to check your code.
Click on OK to close the window.
Click on OK to close Tool Preference.
You shoul done. You may have seen your tool bar flash. The new icon should be there somewhere.
Click on it to test.
I just tried it and it worked for me.
