I now have exactly the same problem and I wonder?
Question for Morten: did this ever get resolved.

Your question and Morten’s problem will be extremely hard to solve. Cookies are hard to deal with when you are working cross environment.

Rolf, thankyou for your interest. I was beginning to wonder if there was anyone out there!.There are 2 cookies involved, the first is a persistent cookie from the browser, the 2nd is a session cookie generated by the DSAPI filter on successful authentication. Everything works fine until a browser without either cookie accesses the server. The DSAPI filter correctly detects this situation and passes the event back to the HTTP task by returning kNotAuthentic and KFilterNotHandled. The next access from a user with a valid session cookie again is correctly detected by the filter but the server ignores it and does not authenticate the user as requested. After a few minutes (of doing nothing) everything returns to normal.
