Data disappeared

I have two multiple value fields and both are editable.On the same button i hav a button called add. when i press add button another form gets open and in that again i have two fiels and where i m entering data. after putting the data this form this data shud display in the parent form. But when i enter the second data the first data gets diappeared from the parent form.
