Client Menu Disappearing

I am developing an application that with a specific form, it will remove the menu (File, Edit, View, etc.) on the Notes client. This happens regardless of whether it is opened, created, or edited. I have never seen this happen before with any of my other forms.

I was wondering if there is specific Lotus Script, @Formula that could do this.

Subject: Corrupt form

Only yesterday, my colleague told me he had the exact same problem some time ago.It was caused by a corrupt form.

Create a new form, select all and copy/paste, copy actions and functions and the new form will be ok.
