Can a design change break/remove embedded OLE objects?

I have an application that uses Word Objects in a Lotus Notes document. Double-clicking on the Word Object opens Microsoft Word.

The users create a new Lotus Notes document, click on the Word icon, and enter information on the Word Document. They close Word and save and close the Lotus Notes document. They can go back into that Lotus Notes document, click on the Word icon again, and the text that was entered in the Word document is visible.

This was working for a few years. I recently made a design change to the Form that the Lotus Notes document is created with. The change was to convert an existing table to a tabbed table - not removing the Word Objects on the Form.

The reason I ask this is now the users cannot retrieve the Word documents on those Lotus Notes documents – they open without any text on them.

I have made design changes in the past on this Form, so what is different this time?

Or am I looking at the wrong thing - the design? Should I be looking at the server or something else?
