There is not a simple answer to this.
BlackBerry does support a “single” mail and BES server with their smaller editions (less then 15 or 30 users) because it is recognized that the cost of a second piece of hardware, licenses, etc was too much to ask for a small install. If you are rolling out a large install (30 - 250 users) then yes a separate server with Domino license (unless you are Express which has no limits on licenses) is required since the BES sits on top of a Domino server and uses NRPC to communicate with the mail servers.

The Blackberry for Domino requires it to be installed on the same box / OS as a Domino Server. This does not mean you need to install another server but it would be advisable. This is because the Blackberry Manager will also need SQL access. The load on your mail server would increase and having a Blackberry server alone increases connections and reads to your server by 1.5 connections per user.
It would be advised for you to have a seperate BES server.
